MBSA 1.2.1 found a WinXP SP1 PC on our system that had MSXML 3.0 SP3. (Our
company does not permit SP2 yet.) When I ran the MSXML SP5 MSI, the installer
finished OK but I did not get a restart alert. When I ran MBSA again, it
still said the PC had MSXML 3.0 SP3. I tried unregistering msxml3.dll and
then running the MSXML 3.0 SP5 MSI, but I still had the same problem. I
haven't had this happen with any other PC on my system.

Does anyone have any ideas why the update isn't working on this PC?

Try to reboot afterwards. Never change the name of a .msi-file. If you do
that, it may fail. You have to uninstall it with the same misspelt name or
try to remove the file from the installer-cache and other
installer-databases to be able to re-install it again.

Else if you use SQL 2000, install SP4 and it will have MSXML 3.0 SP6:

SQL Server 2000 SP4:
a.. Includes a new version of MSXML version 3.0 SP6
